<br />200800227 <br /> <br />weeks in the summer starting in 2004. In the year 2005 and continuing thereafter, the Petitioner <br />shall elect the 14 consecutive days by notifying the Respondent in writing no later than May 1 ~t or <br />forfeit such visitation. The holidays shall be divided pursuant to Wilson v. Wilson, attached an <br />incorporated herein. Exchanges for visitation shall occur in Chapman, Nebraska at a public facility <br />there. <br /> <br />6. Beginning June 1, 2004 Petitioner is Ordered to pay child support in the amount of <br />$512.44 per month for three children, $439.98 per month for two children and $308.72 per month <br />for one child.. Said payments are to be made to the NEBRASKA CHILD SUPPORT PAYMENT <br />CENTER, P.O. Box 83307, Lincoln, Nebraska 68501-3307. Said support shall continue until the <br />child(ren) reaches age 19, becomes emancipated or until further Order of the Court. Delinquent <br />child support shall accrue interest at the rate of3.0080%. <br />7. The Petitioner shall meet her child support obligation as set forth above by means of <br />income withholding through her place of employment. Such withholding shall commence <br />immediately beginning the June 1st, 2004, or as soon thereafter as the employer may initiate such <br />income withholding proceedings. A delay in withholding shall not release the Petitioner from <br />making child support payments as required hereunder. The Clerk of the District Court is hereby <br />directed to provide notice to the employer of withholding requirements. In the event the Petitioner <br />fails to pay any child support payments as such failure is certified each month by the Nebraska Child <br />Support Payment Center in cases in which court ordered support is delinquent in an amount equal <br />to the support due and payable for a one month period of time, the Petitioner may be required to <br />appear in court on a date to be determined by the Court and show cause why such payment was not <br />made. In the event that the Petitioner fails to pay and appear as ordered, a warrant shall be issued <br />for the Petitioner's arrest. <br />8. The Petitioner shall provide health insurance if available through her place of work The <br />Respondent shall pay the first $480.00 of un-reimbursed medical expenses for the children after <br />.which each party shall be responsible for one-half of said expenses. <br />9. Each parent shall have full and equal access to the education and medical records of the <br />minor children; and each parent may make emergency decisions regarding the health and safety of <br />the children while they are in the parents' physical custody pursuant to the terms of this Decree. <br />10.
