200607616 Send to: Nebraska Child SuppOli Payment Center PO Box 83307 Lincoln, NE 68501-3307 Attached hereto is a child support worksheet which shows the calculation of child support based upon the income of the parties at the time of this dissolution. The Petitioner is required to fumish the Clerk ofthe District Court of Hall County, Nebraska, with his address, telephone number, social security number, name of his employer, and all other infoffilation relevant until the judgment for child support entered herein is paid in full. The Petitioner is further ordered to advise the Clerk of the District Court of any changes in such infonnation until the judgment has been fully paid. The Petitioner is required to execute appropriate income withholding documents pursuant to Section 43-1718 of the Nebraska Revised Statutes, and file with the Clerk of the District Court within thirty (30) days of the date of this Decree, notification that he has filed the appropriate withholding documents with his employer. The Petitioner shall be subject to contempt proceedings being initiated for failure to file either the appropriate income withholding documents with the employer or the notification of filing with the clerk. If, because of a change of employment all or a portion of the child support due in anyone month is not withheld from income, it is the Petitioner's obligation to pay such support in full directly to the Nebraska Child Support Payment Center. In the event the Petitioner fails to pay any child, spousal support or medical payment, as such failure is certified each month by the District Court Clerk in cases in which court-ordered support is delinquent in an amount equal to the support due and payable for a one-month period of time, Petitioner may be required to appear in court and show cause why such payment was not made. In the event the Petitioner fails to pay and appear as ordered, a warrant shallbe issued for his arrest. Delinquent child support shall accrue interest at the rate of 3.027 % per annum.
