202008365 <br />decisions affecting the health or safety of the minor children while the children are in the physical <br />custody of such parent. In addition, the Defendant will make available to the Plaintiff the schedule <br />of the minor children's school activities, be it teacher's conferences, Christmas programs or other <br />special events that the minor children participate in at school as well as any schedules as to sporting <br />activities that the minor children participate in and make those available to the Plaintiff in advance <br />of the activity date. <br />6. The Defendant shall pay child support for the benefit of the minor children at the rate of <br />$588.00 per month for two (2) children; and $413.00 per month for one (1) child with child support <br />payments commencing June 1, 2008, and continuing in a like amount on the first day of each month <br />thereafter until each child is emancipated, dies, reaches the age of majority (19 years of age) or <br />further order of the Court. All sums are to be paid through the Nebraska Child Support Payment <br />Center. <br />Defendant shall receive an automatic 50% reduction in child support during the month of <br />August of each year. Said abatement will be automatic unless the Defendant produces an Affidavit <br />stating that the four week visitation did not occur in any summer in which case a hearing shall be <br />scheduled. <br />Attached hereto is a child support worksheet which shows the calculation of child support <br />based upon the income of the parties at the time of this dissolution. <br />The Plaintiff is required to execute appropriate income withholding documents pursuant to <br />Section 43-1718 of the Nebraska Revised Statutes, and file with the Clerk of the District Court <br />within thirty (30) days of the date of this Decree, notification that he has filed the appropriate <br />withholding documents with his employer. The Plaintiff shall be subject to contempt proceedings <br />being initiated for failure to file either the appropriate income withholding documents with the <br />employer or the notification of filing with the clerk. <br />If, because of a change of employment all or a portion of the child support due in any one <br />month is not withheld from income, it is the Plaintiffs obligation to pay such support in full directly <br />to the Nebraska Child Support Payment Center. <br />In the event the Plaintiff fails to pay any child support or medical payment, as such failure <br />is certified each month in cases in which court-ordered support is delinquent in an amount equal to <br />Page 02 of <br />7 <br />
