ATTACHMENT "A" (01552 5. ApPlication To Develop A Tower. Prior to commencement of development or construction of a tower. an application shall be submitted in triplicate to the Buildina Department for a Tower Development Permit and shall include the followina: a) The name. address and telephone number of the owner and if applicable. the lessee of the tract of land upon which the tower is to be located. Applicants shall include the owner of the tract of land and all persons havina an ownership interest in the proposed tower. All applicants shall execute the application. b) The leaal description of the parent parcel and the leased parcel (if applicable) and address of the tract of land on which the tower is to be located. c) The names. addresses and telephone numbers of all owners of other towers or useable antenna support structures within a three (3) mile radius of the location of the proposed tower. includina publicly and privately owned towers or structures. d) A propaaation map and inventorv of all existina towers in excess of one hundred (100) feet located within a three (3) mile radius of the proposed tower location. includina specific information on the owner. location. heiaht and desian of each tower. e) An affidavit attestina to the fact that the applicant has made diliaent but unsuccessful efforts to obtain permission to install or collocate the applicant's telecommunications facilities on a tower or useable antenna support structure within a three (3) mile radius of the proposed tower location or written technical evidence from an enaineer that the applicant's telecommunications facilities cannot be installed or collocated on another tower or useable antenna support structure within a three (3) mile radius of the proposed tower location. f) A notarized statement by the applicant as to whether construction of the tower will accommodate colocation of additional antennas for future uses and. if colocation can be accommodated. a specific description of the type and number of additional antennas and the enaineerina reauirements for colocation. a) A notarized statement by the applicant as to whether the applicant aarees to permit the colocation of the County's emeraency service antennas as provided in section Section Subsection 23. h) Written technical evidence from an enaineer that the proposed tower will meet the Buildina Code. all other construction standards set forth by the County Zonina Resolution and federal and state law and applicable ANSI standards. j) Color photo simulations showina the proposed location of the tower with a photo- realistic representation of the proposed tower as it would appear viewed from the nearest residential zoned property and nearest roadway. street or hiahway. j) Arial photo showina the proposed location of the tower and lands within a three mile radius of the proposed tower. k) Descriptions and diaarams of the proposed tower. telecommunications facilities and/or antenna. manufacturer's literature. appurtenances such as buildinas. driveway. parkina area and fences or other security enclosures with sufficient detail to allow persons reviewina the application to understand the kind and nature of the proposed facilitv.
