2011057�6 <br />(g) All decisions affecfi�ng the chifdren's growth and development, including, but <br />not limited to, choice of schoo(, course of study, exfient of �ravei away from home, <br />inciuding travel autside of the State of Nebraska, chaice Qf camp, majQr medical <br />trea�ent, ceiigious education, iessons, psycho#herapy, psychoanafysis, counseling or <br />fike treatment, part or ful{-time employrrient, purchase or operation of a motor vehicle, <br />specialiy hazardous sports or activi�ies, con�raception and sex education, and decisions <br />relating to actual or patenfiial fitigafion invoiving the child or as a benefiiciary, ather than <br />custody, shafl be cortsid�red ma}or decisions and shafi be cansidered and discussed in <br />depth and agreed to by both the Plair�tiff and the Defiendant. <br />(h) Both parties shall jointiy discuss all school and e�fa-curricufar activities that <br />the minor children wish to �artici�afie in with the other par�y before making any <br />commitman#s regarding such schooi and extra-curricular:activi�ies. in addi�ion, bofh <br />parties shall be alfowed to attend, and participat� in, if a�propriate, aU school and extra- <br />curricular activi�ies, con#erences and programs. Both parties shal[ promptly inform the <br />other party of such schoof and extracurricular activities, conferences and programs as <br />soon as they are made aware of �the same so as to faciii�afie the o#her parties' <br />participation in the same. <br />(i) The Piainfiiff acknowfedges that she cannot remove the minar children from <br />the State of Nebraska an a permanertt basis without obtaining the permission �of the <br />Defendant and the Distric� Court of Hafl County, Nebraska. <br />(j) Both parties agree fio discuss any routine medical examinations neede�, if not <br />of an emergency nature, with the other party prior #a rrzaking said appointments so as to <br />enable the other party to participate in such medica( examinations. Thereafter, both <br />parties agree to ir�form the other party of #he nature o# the medicaf examinations, the <br />diagnosis made and the treatment prescribed, if the other party was not abie to attend <br />said medical examinations with the minor children. tn adcli�ion, both parties agree to <br />share any and ail medical reports and progress reports received from any health care <br />providers and/or school personnef regarding the curr�nt sta�us of the minor chifdren's <br />health with the other party and also allaw the ather,party input into any fufure decisions <br />needed to be made regarding such reports. Pursuant �o NEB. REV. STAT. �2-381, <br />both parties acknaw{edge that they have been advised and understand that, under <br />ldebraska law, regardiess of the determination of the Court relatirtg fo custody of fhe <br />minor chiidren, (a) each parent shall con�inue #o have fulf and equal access to the <br />education and medical records of the minor chif�ren unless the Court orders to the <br />contrary and (b) either parent may make emergency dec�sions affecting the health or <br />safety of the minor children whiie fihe minor chiidren are in the physica! custody of such <br />parent. <br />(k) Phone ca(1s of a rou�ine nature by one party to the minor chifdren while in the <br />physical custody of the other party sha(( be at reasonable tirnes and far reasonable <br />5 <br />Pag�_�___of t � <br />
