Respondent David Christ Davis shall pay support for the child of the marriage in the amount of $325 per month, beginning forthwith and continuing on the fifth day of each month until said child enters the military service, attains the age of nineteen years, is no longer dependent upon the custodial parent for support, marries or dies, whichever occurs first; or until further arder of the court. Said support payments shall be by direct deposit to Petitioner's savugs account from Respondent's disability check. Upon receipt of each payment, Petitioner shall mail - ---a-receipttorthe Clerk-ofthis-court. _ ... _ _ .. 15. Attached to this decree and made a part hereof by reference is a copy of the "Basic Net Income and Support Calculation Worksheet" as promulgated by the Nebraska Supreme Court, showing the respective incomes of the parties and the figures calculating the amount of child support due according to the Nebraska Child Support Guidelines. 16. All child support payments shall become delinquent the day after they are due. Interest, computed as simple interest, shall not accrue until 30 days after the payments are delinquent. 17. In the event Respondent fails to pay any child support, medical support or spousal support payment, as such failure is certified each month by the District Court clerk in cases where court-ordered support is delinquent in an amount equal to the support due and payable for a one-month period of time, Respondent may be required to appear in court on a date to be determined by the court and show cause why such payment was not made. In the event that Respondent fails to pay and appear as ordered, a warrant shall be issued for his arrest. 1 S. Each party is required to inform the Clerk of this court whether he or she has access to employer-related health insurance coverage and, if so, to furnish the health insurance policy information, including the name of his or her health insurance provider, together with the number of the policy and the address at which claims are to be submitted. 19. Respondent is ordered to furnish the clerk of this court with his current address and telephone number; his social security number; the name and address of his emplayer, if any, and any other information the court shall deem relevant until the judgment for support shall be paid in full. Respondent shall also immediately advise the clerk of this court in writing of any changes in any of the information required by this paragraph, until the child support judgment shall be paid in full.
