<br />~\ <br /> <br />200706446 <br /> <br />minor children; and each parent may make emergency decisions regarding the health and safety of <br />the children while they are in the parents' physical custody pursuant to the terms of this Decree. <br />13. Each party shall maintain health insurance for the benefit of the minor children as it <br />should become available through their respective places of employment. Any medical expenses not <br />covered by insurance incurred by the minor children shall be shared equally between the parties. <br />14. The Plaintiff is ordered to furnish to the Clerk of the District Court of Hall County <br />Nebraska and to the Nebraska Child Support Payment Center her address, social security number <br />and such other information that the Court may require from time to time. The Plaintiff shall also <br />advise the Clerk of the District Court of Hall County, Nebraska, and the Nebraska Child Support <br />Payment Center in writing, of any change of employment, address or telephone number between the <br />time of the entry of this Decree of Dissolution and the payment of the Judgment for Child Support <br />entered herein in full. In addition, both parties are hereby required to keep the Clerk of the District <br />Court informed of the name of their health insurance carriers, if any, available through their place <br />of employment, together with the number of the policy and the addresses at which claims under such <br />policies are to be submitted. Each party shall comply with this requirement in regards to health <br />insurance from the date of this Decree until such time as the payment of the judgement for child <br />support entered herein is made in full. Failure to comply with this Order shall be punishable by <br />contempt. <br />15. This Decree will become final thirty (30) days from the date hereof or upon the death <br />of either of the parties hereto, except for purposes of appeal provided that the parties are not free to <br />remarry for a period of six months from the date hereof; and provided further, that health insurance <br />policies maintained by either spouse may be continued for the other spouse for six (6) months from <br />the date hereof. <br /> <br />16.
