~\ 200706446 minor children; and each parent may make emergency decisions regarding the health and safety of the children while they are in the parents' physical custody pursuant to the terms of this Decree. 13. Each party shall maintain health insurance for the benefit of the minor children as it should become available through their respective places of employment. Any medical expenses not covered by insurance incurred by the minor children shall be shared equally between the parties. 14. The Plaintiff is ordered to furnish to the Clerk of the District Court of Hall County Nebraska and to the Nebraska Child Support Payment Center her address, social security number and such other information that the Court may require from time to time. The Plaintiff shall also advise the Clerk of the District Court of Hall County, Nebraska, and the Nebraska Child Support Payment Center in writing, of any change of employment, address or telephone number between the time of the entry of this Decree of Dissolution and the payment of the Judgment for Child Support entered herein in full. In addition, both parties are hereby required to keep the Clerk of the District Court informed of the name of their health insurance carriers, if any, available through their place of employment, together with the number of the policy and the addresses at which claims under such policies are to be submitted. Each party shall comply with this requirement in regards to health insurance from the date of this Decree until such time as the payment of the judgement for child support entered herein is made in full. Failure to comply with this Order shall be punishable by contempt. 15. This Decree will become final thirty (30) days from the date hereof or upon the death of either of the parties hereto, except for purposes of appeal provided that the parties are not free to remarry for a period of six months from the date hereof; and provided further, that health insurance policies maintained by either spouse may be continued for the other spouse for six (6) months from the date hereof. 16.
