APPLICANT STATES:
<br />1. Applicant, as nominated Personal Representative in
<br />Decedent's Last Will and Testament, is an interested person as
<br />defined by the Nebraska Probate Code section 30- 2209(21).
<br />2. Name of Decedent: DOROTHY J. LEPP
<br />Date of Death: October 28, 2001
<br />Age at Date of Death: 86 years
<br />Domicile at Date of Death: Grand Island, Hall County,
<br />Nebraska
<br />3. Venue for this proceeding is proper in this county
<br />because the Decedent was a domiciliary of this county at the date
<br />of death.
<br />4. No Personal Representative has been appointed in this
<br />state or elsewhere.
<br />5. Applicant has not received a Demand for Notice and is
<br />unaware of any demand for notice of any probate or appointment
<br />proceeding concerning the Decedent that may have been filed in this
<br />state or elsewhere.
<br />6. The Decedent's Last Will and Testament, dated April 21,
<br />1993, with Codicil dated July 29, 1993, accompany this Application
<br />and are hereby filed with this Court and offered for probate.

<br />7. The Applicant, to the beat of his knowledge, believes the
<br />Will dated April 21, 1993, with Codicil dated July 29, 1993, to
<br />have been validly executed, and after the exercise of reasonable
<br />diligence, the Applicant is unaware of any instrument revoking the
<br />will and Codicil and believes that said Will with Codicil
<br />constitute Decedent's last Will.
<br />8. Applicant nominates himself, Douglas Denman, Alda,
<br />Nebraska, as Personal Representative, who is qualified and whose
<br />status of priority is as nominated Personal Representtive in
<br />Decedent's Last will and Testament.
<br />There are no other individuals having prior or equal
<br />right to appointment.
