STAKING SHEET- UNDERGROUND Company: OPTK Networks Exchange: Beatrice CONSTRUCTION PROJECT NOTES 1. The State. County, City personnel shall be notified a minimum of forty eight (481 hours in advance of initial project construction. 2. The utility company and the contrna t. (s) are responsible to have at least one person on the job at all times, that Is 'site supervisor' certified. 3. The contractor shall adhere to the current permit and contract specifications. 4. The locations of ail aerial and underground utility facilities may not be indicated on the prgea plans. The contractor is required to contact the Nebraska and Colorado 811 one call notification center (dial 81 1) forty-eight (48) hours poor to working in the city right of way. No excavation will be permitted in the area of the underground utilities until all facilities have been located and identified to the satisfaction of all parties. The contractor shall be responsible for protection of at underground and aenal utilities and infrastructure. 5. Whenever underground facilities are in dose proximity to the proposed pathway location or whenever the vertical location of the underground facility is unknown. the contractor shall use speoal measures to determine the locations of such undergroud facilities. a If the contractor is conducting the excavation by digging from the surface. when in a dose proximity o the undegrorrd facilities, the contractor shall use hand digging. hydro excavating, or air excavating to locate such facility. b. If the contractor is conducting the excavation by tunneling or boning, the contractor shall determine the vertcal location of the underground facility by potholing or any other method approved by the director of public works and utilities_ Project plans shell dimension new facilities being installed. 7. Unless agreed to in suiting in advance the depth of instated facilities n city and county row shall be at a minimum as follows: a. Forty-eight (48i inches in soil b. Forty-eight (48) inches below a projected slope from the flaw line of a drtcn at a three (3) honzontal and one (1) vertical slope. a Forty-eight 1481 inches under a roadway measured from the surface of said roadway to the op of the installation. d. Salty (60) inches under a storm water or creek charnel design bottom or pipe, ar o e. Manton a minimum of twenty -our (24) inches of vertical and Innisuntel spacing from existing utilities. f. Be located as far from the existing or proposed curb line as possible to avoid potential future conflicts 8. Al non -street surfaoe materials that are disturbed by excavation and backfaing operations shag be replaced and restored_ 8. City and county street surface materials disturbed shall be backfilled, tamped and repaved by city or county forces or approved palm° contractor 10. If sidewalk, wheel chair ramps or any portion of the pedestrian facilities are removed or closed, a pedestrian detour shall be established and mainyned during the time of the closure and the entire pedestrian facility shall be restored within seventy-two (72) hours, °Ness the contractor provides a hard surface alternate route approved by the city or county 11. Al pedestrian detours and any reconstruction of pedesrian facilities shalt meet current Americans with Disability Act (ADA) standards and specifications. 12. At pedestrian detours shall be part of an approved Traffic Control Plan (TCP( 13. Al holes made in the streets for the purpose of performng vacuum excavations to locate underground utilities shall be restored to their original condition with the reinstated core flush with and in the onginal orientation as the existing surfaoe matching existing pavement surface appearance. 14. When the contractor is not actively working; at equipment. fencing. debns, etc. Shall not be placed within the triangular area required for sight ttstanoe of vehicles exiting or entering an adjacent property Or mtef520900. 15. At excavations shah be adequately fenced and covered when contractor is not present or project site left unattended. 18. No lane closures allowed on arterial streets during AM and PM peak haws. 17. AM work zone traffic control. inducting pedestrian control measures. shall be in compliance with the MUTCD and ADA. 18. The contractor shall mant3n a set of `as twin' plans on site with dimensioning. These plans shall be presented upon request. 19. Final `as -built' plans shall be filed with city andfor county wiMwf 30 days of the completion of the project. 20. The contractor shall follow construction standards pertaining to the lateral corridor facility placement and minimum depth requirements and clearance requirements of other facilities. 21. A current State. County. or City permit shall be maintained on each work site by the contractor. The contractor shall present such perrnit upon request. 22. Any field adjustments to installation of facilities, which vary from the plans that have been submitted and approved during the pemut application process, shah oompty with the following: a. The contractor must stop work immediately and contact the facility owner W the proposed changes. b. The facility ~tees representative shall contact the city, county or state nght"of-way construction inspector and provide them with the details. c. The ROW inspector will make a determination on tow to proceed. This determnaton may range from approval over the phone or via email to delaying the project until plans can be updated and appropriate review can be conducted. 23. Stake mounted waning signs and marker posts (BM53) to be placed where appropnate. 24. All conduit and fiber is to be installed at the back of the roadway ROW unless otherwise noted. 25.
