CITY OF GRAND ISLAND NOTES <br />"No work in this area one week prior to or during the Nebraska State Fair (August 23 -Sept 2). All open excavations shall be filled during the Nebraska State Fair as <br />there are many pedestrians during this time. <br />The majority of this run is open ditch so all open excavations shall have erosion control. All ditch flow lines shall be restored to the same condition as poor to work <br />beginning. This may require additional seeding and placement of erosion control matting. <br />When filling open excavations for vaults or hand holds the top soil shall be separated and used to make the final lift of backfill (sandy soil placed in the hole first). <br />Please review the plan set and ensure all the below can be met. These notes shall also be incorporated onto the plans. <br />The Grand Island Streets Division shall be contacted prior to work beginning at 308-385-5322. <br />A Notice to Proceed with work in the ROW will not be given until a pre -construction notice has been done; the City of Grand Island permit number is required when <br />requesting the Notice to Proceed. <br />Any keyhole potholing locations in the roadway shall be approved by the Streets Division and shall be filled within five days of potholing. <br />No handholds or vaults shall be placed in the flowline or sloped bank of drainage ditches. If it has to be placed on sloped bank the final grade of hand hold or vault Id <br />shall match that of the sloped bank. <br />Erosion control shall be in place for all open excavations adjacent to drainage ditches. All drainage ditch (towlines must be restored before job is done. <br />No handholds, vaults, or manholes shall be placed in the ADA accessible area of the sidewalk. <br />Any handholds, vaults or manholes placed M the sidewalk shall match the existing slopes of the sidewalk and shall not be or creating tipping hazard for pedestrians. <br />Any sidewalk removals 8 replacements shall be approved by the City of Grand Island prior to removal. Restoration of sidewalk shall be done within 5 days of removal <br />and shall be inspected by the City of Grand Island Public Works Department. <br />Any street or sidewalk closures require a 72 -hour notice to the City of Grand Island Public Works Department at 308-385-5444. <br />Contractor will be required to a locate before any physical work is to take place. Locate flags shall be removed after the 'excavator' has determined the work is <br />complete. <br />A ROW inspection will be completed once work is done. All ROW shall be restored to the same condition as prior to the work taking place. <br />Where CROSSING SIGNAL INFRASTRUCTURE <br />Conduit shall not be placed between the City's traffic signal cabinet and connecting traffic signal conduit/poles. Conduit must be offset a minimum of 5' from the base <br />of any traffic signal cabinet, traffic signal pole, or traffic signal pull box. <br />IF SIGNAL CONDUIT/WIRE HAS TO BE CROSSED THE NEW LINE SHALL BE A MINIMUM OF 6' DEEP WHERE CROSSING AND THE EXSTING SIGNAL <br />CONDUIT SHALL BE POTHOLED AND VERIFIED ON-SITE PRIOR TO BORING." <br />MAINTAIN Y OF HORIZONTAL SEPARATION <br />IF PARALLELING WATER MAINS & FIRE <br />HYDRANTS.
