The Department reserves the right to <br />cancel the contract if these special conditions are not submitted within this specified time frame. <br />§2.01 NAHTF Program Grantee Information. <br />The Recipient must complete a NAHTF Program Grantee Information form within the <br />AmpliFund grant management system for Departmental review as prescribed. <br />§2.02 NAHTF Authorization to Request Funds. <br />The Recipient must complete a NAHTF Authorization to Request Funds form signed in blue ink <br />and uploaded to the AmpliFund grant management system as prescribed by the Department. <br />§2.03 Fair Housing. <br />The Recipient must complete a NAHTF Fair Housing form within the AmpliFund grant <br />management system identifying the project's fair housing representative's name and contact <br />information. The Recipient must submit a description of the action it will take during the course <br />of the Project to fulfill any requirements to affirmatively further fair housing. Following the <br />completion of such action, the Recipient must also submit documentation demonstrating the <br />action that was actually taken, including the details of such action (e.g., when the action <br />occurred, who participated, who benefitted, etc.). The requirement to submit documentation <br />demonstrating the action that was actually taken needs not be submitted within the time frame <br />for completion of the special conditions, but such documentation must be submitted prior to <br />closing of the award. <br />§2.04 Nebraska Uniform Residential Landlord and Tenant Act. <br />The Recipient must submit their proposed tenant selection process and proposed lease <br />agreement. The Recipient shall comply with the State of Nebraska's Uniform Residential <br />Landlord and Tenant Act (Neb. Rev. Stat. §§76-1401 – 76-1449). It is the Recipient's <br />responsibility to seek advice from legal counsel to ensure compliance with the Uniform <br />Residential Landlord and Tenant Act for matters including, but not limited to, tenant selection <br />and lease agreements. <br />§2.05 Program Guidelines <br />The Recipient must submit program guidelines, specific to this award only. The program <br />guidelines must be in compliance with the requirements of the Program and any other policies <br />issued by the Department. <br />§2.06 Rental Project Pro Forma. <br />The Recipient must submit a rental project pro forma ("Pro Forma") for the Department's <br />approval. The Pro Forma must be in compliance with the requirements of the Program and any <br />other policies issued by the Department. The Pro Forma must include unit information (including <br />unit rents), a development cost schedule, annual operating expense information, sources and <br />NAHTF 23-TFRH-34010—Page 5 of 18 <br />
