202103518 <br />Section 12 — Voting Rights <br />My Attorney -in -Fact may exercise voting rights and execute proxies respecting securities or <br />membership in any condominium, tenant or subdivision association, savings and loan association, <br />credit union, mutual insurance company, cooperative or other organization. <br />Section 13 — Services <br />My Attorney -in -Fact may contract for assistance in legal, tax, bookkeeping and investment <br />matters; to obtain services to manage and maintain real or personal property; and to obtain <br />housekeeping, nursing, custodial care and other personal services. <br />Section 14 — Medical and Health Care Services <br />My Attorney -in -Fact may obtain and discontinue medical services from health care providers, <br />including examination, X-ray, laboratory work, diagnosis, medication, surgery, hospitalization, <br />convalescence and physical therapy; to obtain dental, vision and prosthesis devices; to consent, <br />withhold consent or withdraw consent to any medical treatment recommendation; to receive <br />reports and information respecting my physical and mental condition or medical treatment; to serve <br />as my personal representative for all purposes of the Health Insurance Portability and <br />Accountability Act (HIPAA) (Public Law 104-191); and to execute releases and waivers of my <br />Protected Health Information (PH) as defined by HIPAA. My Attorney -in -Fact shall have the <br />power and authority of a designated representative for all purposes under the Health Insurance <br />Portability and Accountability Act of 1996 ("HIPAA"), 42 U.S.C. Section 1320d and 45 C.F.R. <br />Parts 160-164, and is authorized to execute releases and other documents necessary to obtain <br />disclosure of individually identifiable health information, medical records, and patient files, <br />including psychotherapy notes. This information includes, but is not limited to, any written <br />opinion or assessment of my decision making capacity. This authorization and release apply to all <br />information protected by HIPAA and shall only expire if I revoke this power of attorney. <br />Section 15 — Retirement Plans <br />My Attorney -in -Fact may establish, maintain, terminate, make contributions to, receive <br />distributions from, make withdrawals from, or designate beneficiaries to, any profit -'sharing plan, <br />pension plan, employee stock ownership plan, deferred tax plan, or other retirement plan in which <br />I am a participant or a beneficiary. <br />Section 16 — Access Digital Media and Accounts <br />My Attorney -in -Fact may access and obtain all digital or electronic data that may be stored on my <br />desktop, laptop, tablet, or other computer, peripheral drive, storage device, mobile telephone or <br />any similar device, including without limitation, all internet accounts (including e-mail accounts, <br />iTunes, financial reports and archives of the same), on-line photographs and videos, on-line music, <br />on-line documents, all licenses to on-line items and software, social network accounts, domain <br />registrations, DNS service accounts, web hosting accounts, on-line stores, tax preparation service <br />accounts, file sharing accounts, computer backup processes, and user passwords and other security <br />access to any of the foregoing, and all similar digital items which currently exist or may exist as <br />technology develops. <br />Durable Power of Attorney <br />HAZEL MARGENE STUEVEN Page 7 of 14 <br />
