11 <br />eye glasses, contact lens, prescription and counseling expenses incurred on <br />behalf of the minor children. Once that threshold is reached, the parties shall <br />divide the unreimbursed expenses with the Plaintiff paying thirty percent (30 %) <br />and Defendant paying seventy percent (70 %) of each child's non - reimbursed <br />reasonable and necessary health care costs. Plaintiff shall provide Defendant <br />with a written itemization of the amount expended, including the amount <br />expended to reach the $480.00 threshold, accompanied by appropriate <br />supporting documentation within forty -five (45) days of receipt or she shall be <br />deemed to have waived such right to reimbursement. Thereafter, Defendant <br />shall reimburse Plaintiff for his seventy percent (70%) share within fifteen (15) <br />days. <br />12. Alimony: The Defendant shall pay alimony through the Nebraska Child <br />Support Payment Center, P.O. Box 82600, Lincoln, NE 68509 -2600 in the <br />amount of $1.00 per year for a period of five (5) years. The parties agree that the <br />awarding of the $1.00 in alimony is for the sole purpose of securing the payment <br />of the equalization payments as set forth further in this Decree. In the event that <br />the Defendant defaults, discharges or otherwise fails to pay any of the amounts <br />due under this Decree, Plaintiff shall be entitled to modify alimony in an amount <br />equal to the remaining amounts due under the Decree and the property <br />settlement agreement.
