<br />STEPHEN J. DOUGLAS and PATRICIA A. KING being first duly sworn upon oath,
<br />depose and state that:
<br />1. Marion D. Larsen created the Marion D. Larsen Revocable Trust by Agreement
<br />dated November 9, 1992 which he subsequently amended and restated on various dates with the
<br />most recent amendment and restatement being the Fourth Amendment and Restatement of the
<br />Marion D. Larsen Revocable Trust dated October 18, 2008 which he subsequently amended on
<br />September 5, 2012, December 13, 2012, September 6, 2013 and August 27, 2014 (the "Marion
<br />D. Larsen Revocable Trust ") wherein he designated himself as the initial Trustee and designated
<br />Stephen J. Douglas and Patricia A. King as successor Co- Trustees.
<br />2. From the date of creation of the Trust until September 4, 2016, Marion D. Larsen
<br />served as Trustee of the Marion D. Larsen Trust;
