<br />2. The entity against whose interest in the real estate the lien is claimed is:
<br />Monfort, Inc., a division of ConAgra.
<br />3. The name and address of the claimant is Consolidated Electrical
<br />Distributors, Inc., P.O. Box 22254, 6000 East Evans, Building 2, Suite 260, Denver,
<br />Colorado, 80222.
<br />4. The name and address of the person with whom the claimant contracted
<br />was Rick Hitchler, whom upon information and belief was acting as agent for the owner,
<br />Monfort, Inc., a division of ConAgra, 555 South Stuhr Road, Grand Island, Nebraska,
<br />68802.
<br />5. A general description of the claimant's services performed or to be
<br />performed or materials furnished or to be furnished for the improvement and the
<br />contract price is: removing old electrical switches, installing new electrical switches,
<br />bussing, testing of new switches for upgrade, installing inverted mechanisms and
<br />covers. The contract price was $47,538.88.
<br />6. The amount unpaid, whether due or not, to the claimant for the services
<br />and materials is the sum of $47,538.88.
<br />7. The time the last services or materials were furnished was: December 17,
<br />2000.
