200002054 I have been fully informed of all facts relating to powers of attorney for health care and I understand the consequences of making this appointment of my Agent as my attorney -in -fact for health care. Having considered those consequences I do hereby specifically declare that: (1) I do not desire to have my life artificially prolonged if I am not able to effectively communicate with my family and my doctor and if there is no reasonable expectation that I will recover from any condition and thereafter be able to live without the continuing artificial support. Therefore, I direct that to the full extent allowed by law my attorney -in -fact shall have authority to consent to the withholding or withdrawing of a life - sustaining procedure or artificially administered nutrition or hydration or any other medical treatment from me, and (2) Any provider of medical services may rely conclusively upon any and all decisions, consents, withdrawals of consent and directions made by my attorney -in -fact under the authority contained in this document. In consideration of the medical provider's reliance upon the actions of my attorney -in- fact, I hereby contract to defend, indemnify and save harmless any medical provider who does so rely of and from any and all claims, demands, suits and causes of action which arise from that provider's acts or failures to act as a result of that reliance and brought by me or on my behalf or by any person claiming by, through or under me or as a result of my death. This contract is and shall be fully binding upon my heirs, personal representatives and my estate. DURATION OF POWERS I do hereby revoke any and all Durable Powers of Attorney heretofore executed by me. This is a Present Durable Power of Attorney which becomes fully operative at the time of its execution by me and remains _ operative until - revoked.
