CHILD SUPPORT: Pursuant to the Child Support Calculation attached hereto and approved by the Court, the Defendant is ordered to pay child support in the amount of $814.00 per month for two children, and $560.00 per month for one child, commencing May 1, 2014, and continuing on the first of each month until the children reach age 19, become emancipated, marry, die, or until further Order of the Court. Said payments are to be made through income withholding and shall be sent to the Nebraska Child Support Payment Center, P.O. Box 82890, Lincoln, NE 68501 -2890, with said funds being targeted to the above - referenced case for disbursement to the Plaintiff. 7. The Plaintiff shall not pay alimony to the Defendant and the Defendant shall not pay alimony to the Plaintiff. 8. The Defendant is required to execute appropriate income withholding documents pursuant to Section 43 -1718 of the Nebraska Revised Statutes, and file with the Clerk of thc District Court within thirty (30) days of the date of this Decree, notification that he has filed the appropriate withholding documents with his employer. The Defendant shall be subject to contempt proceeding being initiated for failure to file either the appropriate income withholding documents with the employer or the notification of filing with the clerk. 9. 1f, because of a change of employment all or a portion of the child or spousal support due in any one month is not withheld from income, it is the Defendant's obligation to pay such support in full directly to the Nebraska Child Support Payment Center, P.O. Box 82600, Lincoln, NE 68501 -2600. 10. Delinquent child or spousal support shall accrue interest at the following rate of 2.066 %. 11. Each party is ordered to furnish to the Clerk of the District Court and the Child Support Enforcement Hotline (1- 877 -631- 9973), their address, telephone number, social security number, the name of their employer, and the name of their health insurance carrier, if any, together with the number of the policy and the address at which claims are to be submitted. Each party shall also advise the Clerk in writing, of any change in such information between the time of the entry of the support order and the payment of the child support judgment entered herein in full.
