201502798 <br />11. The court costs are taxed to the Petitioner and each <br />party shall pay their respective attorney's fees. <br />12. It is the finding of the Court, that mandatory income <br />withholding for child support is not required for the reason that <br />the parties have shown good cause not to require such immediate <br />income withholding or that pursuant to written agreement between <br />the parties a satisfactory arrangement has been established for the <br />payment of said support. In the event the Petitioner fails to pay <br />any child, medical, or spousal support payments, as such failure is <br />certified each month by the District Court Clerk in cases in which <br />court ordered support is delinquent in an amount equal to the <br />support due and payable for a one month period of time, the <br />Petitioner may be required to appear in court on a date to be <br />determined by the court and show cause why such payment was not <br />made; and that Petitioner may be subject to income withholding <br />through his place of employment. In the event that the Petitioner <br />fails to pay and appear as ordered, a warrant shall be issued for <br />his arrest. <br />13. Each parent shall have full and equal access to the <br />education and medical records of the minor child; and each parent <br />may make emergency decisions regarding the health and safety of the <br />child while she is in the parents' physical custody pursuant to the <br />terms of this Decree. <br />14. The Petitioner is ordered to furnish to the Clerk of <br />the District Court of Hall County, Nebraska, his address, social <br />security number and such other information that the Court may <br />require from time to time. The Petitioner shall also advise the <br />Clerk of the District Court of Hall County, Nebraska in writing, of <br />any change of employment, address or telephone number between the <br />time of the entry of this Decree of Dissolution and the payment of <br />the Judgment for Child Support entered herein in full. In <br />addition, both parties are hereby required to keep the Clerk of the <br />District Court informed of the name of their health insurance <br />carriers, if any available through their place of employment, <br />together with the number of the policy and the addresses at which <br />claims under such police are to be submitted.
