201403529 5. Trust Irrevocable. The Declaration of Trust provides that only the Settlors shall have the power to revoke, amend, or modify the Declaration of Trust. CAMILE B. SMITH died on January 25, 2014 so no one has the power to revoke, amend, or modify the Declaration of Trust. 6. Identifying Number of the Trust. The Trust's employer identification number is 46-7298798. 7. The Trustees. The initial trustee of the Camille B. Smith Survivor's Trust UTA of Trust. dated August 27, 1997 was CAMILE B. SMITH,who is now deceased and unable to act as Trustee. Under the terms of the above-mentioned Trust,the undersigned, SHERRE DIANE TASSOS and DONNA CARLEEN KLUCK, CAMILE B. SMITH's daughters, are designated to serve and shall serve together as Co-Successor Trustees. SHERRE DIANE TASSOS and DONNA CARLEEN KLUCK, (the"Trustees")are the currently serving and acting trustees of the Trust. Paragraph 20 of the Declaration of Trust states,"If either EARL P. SMITH or CAMILLE B. SMITH shall,for any reason, fail to qualify or cease to act as Trustee,then the Surviving Spouse,together with SHERRE DIANE TASSOS and DONNA CARLEEN KLUCK, or the survivors of them, shall serve as Co-Trustees."This Certification of Trust is being signed by all of the currently serving and acting trustees of the Trust. 8. Beneficiary. The ultimate beneficiary of the property affected by this Trust Certification according to paragraph 7c of the Declaration of Trust is DONNA CARLEEN KLUCK,the Settlor's daughter,if she is then living, and if not, then such share shall be allocated to her issue. 9. Form of Title. At this time,title to the assets of the trust is to be taken in the names of SHERRE DIANE TASSOS and DONNA CARLEEN KLUCK,Trustees of the Camille B. Smith Survivor's Trust under Declaration of Trust dated August 27, 1997. 10. Signature Authority. As the sole currently serving Trustees, SHERRE DIANE TASSOS and DONNA CARLEEN KLUCK,have the power to act on behalf of the Trust and to sign all documents exercising the powers of the Trustee. Under the Trust Agreement,no person other than the sole Trustees is required or needed to sign such documents for them to be effective as to the Trust. 11. Reliance by Third Parties. The Trust has not been revoked,modified,or amended in any manner that would cause the representations contained in this Certification of Trust to he incorrect. This Certification contains a true and correct representation of the terms of the Trust. All third parties dealing with the Trustee may rely on this Certification as a true statement of the provisions of the Trust as of the date this Certification is presented to the third party(regardless of the date of execution of this Certification),unless the third party has actual knowledge that the representations contained in this Certification are incorrect. A third party who acts in reliance upon this Certification of Trust without actual knowledge that the representations contained herein are incorrect is not liable to any person for so acting.
