St. §30- 38,103) <br />SHERRE DIANE TASSOS and DONNA CARLEEN KLUCK hereby present this <br />Certification of Trust, in lieu of providing a copy of the trust instrument, to establish the <br />existence and terms of the trust, as set forth below. <br />1. Purpose of Certification of Trust. This Certification is intended to serve as a <br />"Certification of Trust" under Nebraska Rev. St. §30- 38,103, as amended. Its purpose is (1) to <br />certify the existence of the Camille B. Smith Survivor's Trust UTA of Trust dated August 27, <br />1997 (the "Trust ") and the identity of the Trustee, and (2) to summarize some of the more <br />important provisions of the Trust so that the Trustee can deal with third parties, such as financial <br />institutions, stock transfer agents, brokerage houses, insurance companies, and others, without <br />disclosing the entire Declaration of Trust, which is a private and confidential document. <br />2. Property Affected. This Certification of Trust affects property situated in the <br />State of Nebraska, County of Hall, City of Grand Island, commonly known as 604 Gold Road, <br />Grand Island, and more particularly described as `LOT FIFTEEN (15) in Westgate Subdivision, <br />Grand Island, Hall County, Nebraska," A.P.N. 400 -166 -720. <br />3. Creation of the Trust. The Trust Instrument creating the Camille B. Smith <br />Survivor's Trust was executed on August 27, 1997 in California. The prior Trustee, CAMILLE <br />B. SMITH, died on January 25, 2014. The current acting successor Co- Trustees are SHERRE <br />DIANE TASSOS AND DONNA CARLEEN KLUCK. The Trust continues in existence. <br />4. Powers of the Trustees. The powers and limitations of the Trustees are <br />enumerated in Paragraphs 10 through 15 of the Declaration of Trust, pages 12 -17, attached <br />hereto as Exhibit "A ". <br />Certification of Trust <br />Recordable / Death of Prior Trustee <br />
