Stehlik, after being first duly sworn upon oath would hereby <br />depose and state as follows: <br />1. I represent Cathy Dellwo, the duly appointed and currently acting Personal Representative <br />of the Estate of Betty Nye. <br />2. Prior to her death, Betty Nye retained me to assist her in the following: <br />a. Having her son, David Hadford, appointed as her Power of Attorney under a General <br />Durable Power of Attorney; <br />b. Directing David Hadford, as Attorney -In -Fact, to formally revoke a Transfer On <br />Death Deed that Betty Nye had earlier signed dated on May 3, 2017, and recorded as <br />Instrument No. 201702932 in the office of the Register of Deeds, Hall County, <br />Nebraska, dealing with the following described property, to wit: <br />Lot One (1) in Block Fifty -One (51), Charles Wasmer's <br />3rd Addition to the City of Grand Island, Hall County, <br />Nebraska. <br />
