Campbell and Linda S. Cooper, being first duly sworn upon oath, depose and state that: 1. Gene W. Campbell created the Gene W. Campbell Revocable Living Trust pursuant to Trust Agreement dated of June 27, 1996, as amended February 18, 2003, July 10, 2007 and May 23, 2011 (the "Trust") wherein he designated himself as the initial Trustee and designated Harold E. Campbell and Linda S. Cooper as successor Co-Trustees. 2. From the date of the creation of the Trust until June 8, 2011 Gene W. Campbell served as Trustee of the Trust. 3. Gene W. Campbell died on June 8, 2011 as evidenced by the attached copy of his death certificate. 4. As of June 8, 2011 Affiants executed an Acceptance of Co-Trusteeship accepting the duties as Co-Trustees of the Trust and agreeing to administer said Trust in accordance with the terms of said Trust Agreement and the laws of the State of Nebraska. 5. 6.
