2 oioo���� <br />CONDOMINIUM RIDER <br />THIS CONDOMINIUM RIDER is made this 22nd day of October 2010 <br />and is incorporated inta and shall be deemed to amend and supplement the Mortgage, Deed <br />af Trust, or Security Deed (the "Security Instrument") of the same date given by the <br />undersigned (the "Borrower") ta secure Borrower's Note to EQUITABLE BANK <br />(the <br />"Lender") of the same date and covering the Property described in the Security Instrument <br />and located at: <br />2128 W 6TH ST <br />Grand Island, NE 68803 <br />[Praperty Address] <br />The Property includes a unit in, together with an undivided interest in the common elements <br />of, a condominium project known as: <br />[Name of Condominium Praject] <br />(the "Condominium Project"). If the owners association or other �ntity which acts for the <br />Condominium Project (the "Owners Association") holds title to property for the benefit or use <br />of its members ar shareholders, the Property also includes Barrower's interest in the Owners <br />Association and the uses, proceeds and benefits of Borrower's interest. <br />CONDOMINIUM COV�NANTS. In addition ta the covenants and agreements made in the <br />Security Instrument, Borrower and Lender further cavenant and agree as follaws: <br />A. Condominium Obligations. Borrower shall perform all of Borrower's obligations und�r <br />the Condominium Project's Constituent Documents. The "Canstituent Documents" are the: (i) <br />Declaration or any other document which creates the Candominium Project; (ii) by-laws; (iii) <br />code of regulations; and (iv) qther equivalent documents. Borrower shall pramptly pay, when <br />due, all dues and assessments imposed pursuant to the Constituent Documents. <br />B. Property Insurance. So long as the Owners Associatian maintains, with a generally <br />accepted insurance carrier, a"master" or "blanket" policy on the Condominium Project which <br />is satisfactory to Lender and w hich provides insurance coverage in the amounts (including <br />deductible levels), for the periods, and against Ioss by fire, hazards included within the term <br />"extended caverage," and any other hazards, including, but not limited to, earthquakes and <br />floads, from which Lender requires insurance, then: (i) Lender waives the provision in <br />F[�II�I:II�I�I�I�Zl/ <br />MULTISTATE CONDOMINIUM RIDER - Single Family - Fannie Mae/Freddie Mac UNIFORM <br />INSTRUM ENT <br />Form 3140 1 /01 <br />Wolters Kluwer Financial Services <br />VM P �-$R (0810) <br />Page 1 of 3 Initials: � <br />
