CMH DISTRIBUTING, INC., P.O. Box 409, Elm Creek, Nebraska 68836 is the <br />name and address of the Claimant. <br />4. E.D.I., 220 McPherson Avenue, Republican City, Nebraska, 68971, is the name <br />and address of the person with whom Claimant contracted. <br />5. Claimant furnished and provided drywall for the project. The contract price for <br />such materials was $ 97,761.44 <br />6. $20,293.14 is the amount unpaid to Claimant for said materials. <br />7.
