Diane Bockmann, Trustee, being first duly sworn upon his or her oath, deposes and says: 1. That the Affiant is the successor trustee under the terms and conditions of the Darryl D. Bockmann Revocable Trust dated December 27, 2000 (hereinafter referred to as the "said Trust"). 2, That on June 23, 2009, the Affiant became the successor trustee of the said Trust following the death afDarryl D. Bockmann (also known as Darryl Dean Bockmann) on June 3, 2009, the settlor and initial trustee of said Trust, a certified copy of his death certificate is attached as Exhibit "A", and upon signing an Acceptance to Act as Trustee under the Darryl D. Bockmann Revocable Trust, an original of which is attached as Exhibit "B". 3 , That the Affiant remains the sole trustee of the said Trust as of this date. 4.
