<br />The undersigned James R. Jones, attorney for Gerald
<br />E. Chizek, the duly appointed, qualified, and acting Commissioner
<br />of Labor of the State of Nebraska, does hereby certify that R.T.
<br />Roe, doing business as Phoenix Band, doing business in Grand
<br />Island, Hall County, Nebraska, is indebted to the Unemployment
<br />Compensation Fund of the State of Nebraska for contributions due
<br />on his employment for the second, third and fourth quarters of
<br />1974 in the sum of $79.29, and the first, second, third and
<br />fourth quarters of 1975 in the sum of $235.03 (the fourth quarter
<br />of 1975 has been assessed), and interest in the sum of $125.84,
<br />with interest at the rate of one per cent per month on the sum
<br />of $309.32, from December 14, 1978.
<br />By reason thereof, said Gerald E. Chizek, as Commissioner
<br />of Lobar, on behalf of the Unemployment Compensation Fund, hereby
<br />claims a lien in the sum of $435.15, Frith interest at the rate of
<br />one per Dent per month on the sum of $309.32, until paid, upon .~_,-
<br />and all prcp~rty, real or personal, of said R.T. Roe, doing business
<br />as Phoenix Band, and specifically claims a lien on the following
