<br />MATTHEW MIDDLETON, duly elected Vice-President of Middleton Electric, Inc., a
<br />Nebraska corporation, being first duly sworn upon oath, deposes and says:
<br />1. He is the duly elected Vice-President of Middleton Electric, Inc., a Nebraska
<br />corporation and as said Vice-President verifies this account in writing for and on behalf of said
<br />corporation.
<br />2. Middleton Electric, Inc., by and through its duly authorized officers, entered into a
<br />contract with Jeff Reis, 4145 Manchester Road, Grand Island, Nebraska 68803-1508, to provide
<br />labor and materials for electrical requirements in connection with construction of a duplex upon
<br />the real estate hereinafter described, and the contract price for the improvements to be
<br />constructed upon such real estate by Middleton Electric, Inc. including change orders through
<br />August 28, 2009, is Eight Thousand Seven Hundred and 00/100 Dollars ($8700.00).
<br />3. The labor and materials provided are upon and in respect to that real estate located at
<br />4027/4029 Anna Marie Avenue, Grand Island, Nebraska, being a part of that property legally
<br />described as follows, to-wit:
<br />Condominium Unit Nos. 19 and 20, Narthview Seventh Condominiums, a
<br />condominium in accordance with the Declaration recorded an August 26, 2008, under
<br />Instrument Na. 200807402 of the County of Hall, State of Nebraska, records.
<br />4. The amount unpaid and due to Middleton Electric, Inc. far materials and services
<br />provided in accordance with the invoices and change orders through August 28, 2009, is Faur
<br />Thousand Three Hundred Fifty and 00/100 Dollars ($4,350.00) together with interest thereon as
<br />provided by applicable law.
<br />5. The last day services or materials were furnished was August 28, 2009.
