The name and address of the person with whom the claimant contracted <br /> <br />is Gary Leece and Ron Krouse of the Great Escape, Inc., 3235 South <br /> <br />Locust Street, Grand Island, Nebraska. <br /> <br />5. A general description of the claimant's services performed and <br /> <br />materials furnished for the improvement and the contract price is: <br /> <br />Claimant sold to Gary Leece and Ron Krouse, d/b/a The Great Escape, Inc., <br /> <br />a complete audio and lighting system for an original contract price of <br /> <br />$12,391,64, which included installation. Additional items were purchased <br /> <br />in the sum of $1,417.70 and additional labor was performed for $200.00, <br /> <br />with additional sales tax of $49,62.
