The name and address of the person with whom the claimant contracted IS <br />SemMaterials, L.P. <br /> <br />5. A general description of the claimant's services performed or materials furnished for <br />the improvement and the contract price is: Insulation labor and materials for sewer <br />servIce. <br /> <br />6. The amount unpaid to the claimant for the services and materials is $69,103.00. <br />Interest continues to accrue on the amount unpaid at the rate of 1.5% per month. <br /> <br />7.
