002018 <br />NOTICE OF AMENDMENT <br />TO LEASE AND <br />OPTION TO PURCHASE <br />NOTICE IS HEREBY GIVEN that the undersigned, Garth A. <br />Otis and Anthony Wheeler, as Landlord, executed a ten -year <br />Lease which contains two five -year renewal options demising <br />the real property described below to Med West Health Care <br />Management Corporation, as Tenant. In addition, Anthony <br />Wheeler and Garth A. Otis executed at the same time, as <br />Optionors, an Option granting to Med West Health Care. <br />Management Corporation the exclusive right and option to <br />purchase said demised premises, said option to be exercised <br />and consummated during the calendar year 1991, provided that <br />said Lease has not previously been terminated. <br />Said Lease and Option Agreement have been amended <br />correcting the legal description of the said real property <br />to read as follows: <br />Lots 1, 2, 3, 5, 6 and the Southerly <br />2 feet of Lot 4, in Block 4, and Lots <br />7 and 8, in Block 3, all in PACKER <br />AND BARRIS ADDITION to the City of <br />Grand Island, Hall County, Nebraska, <br />together with the East half of vacated <br />alley adjoining Lots 1, 2, 3 and the <br />Southerly 2 feet of Lot 4, in Block 4 <br />on the West, and together with the <br />West half of vacated alley adjoining <br />Lots 5 and 6, Block 4, on the East. <br />Gar <br />C4,A tq , 6 <br />A.
