Wagoner, Jr., M.D. $ 40,000.00 Name Amount of Note 204 West 23rd Grand Island NE 68801 April 30, 1986 Date of Note For value received, I promise to pay to the order of Internal Medical Associates of Grand Island, P.C. Profit- Sharing Plan, c/o Norwest Bank Nebraska, N.A., the sum of forty thamsard dollars ($40,000.00) with interest at the rate of 7.0 percent per annum, beginning on the 1st day of June, 1986, and payable on the same date of each succeeding month until paid inl. I have a right to pay the full indebtedness prior to maturity. All p _ enrs are to be made at the office of the trustee. Payment shall be made in full on or before the 1st day of May, 1996. Upon default in the payment of any installment, or upon any of the following events, the failure to pay any payment, the insolvency of the borrower, the filing of any petition under the provisions of the Federal Bankruptcy Act, or the issuance of any attachment or execution against any of the assets of the borrower, and at the option of the holder of this note, all monies payable under this note shall be due and immediately payable until payment in full or maturity. Principal and interest shall be payable in lawful ►ropey of the United States. I secure this loan by a pledge and irrevocable assignment of the borrower's vested interest in the Internal Medical Associates of Grand Island, P.C. Profit - Sharing Plan, a copy of which is provided with this rote. I am obligated and liable under this promissory note and hereby consent to renewals, replacements and extension of time for payment hereof before, at or after maturity and consent to the acceptance of security or substituted security for this note and waive presentment, the right to assert any statute of limitations, demand, protest and notice of dishonor and of any renewal or extension of the note and consent to any such renewal or extension. If I do not pay this note when due, I promise to pay all costs of collection and reasonable attorneys' fees incurred by the holder of this note on account of such collection, whether or not suit is filed on this note.
