<br />1. He is the Secretary of Jerry's Sheet Metal, Heating & Cooling, Inc., and as
<br />said Secretary, verifies this account in writing for and on behalf of Jerry's Sheet Metal,
<br />Heating & Cooling, Inc.

<br />2. Jerry's Sheet Metal, Heating & Cooling, Inc., by and through its duly
<br />authorized agents, provided materials and labor for work done on property located at 1405
<br />South Harrison, Grand Island, Hall County, Nebraska, legally described as follows:
<br />
<br />Lot Three (3), in Block Two (2), in Normandy Estates, an
<br />Addition to the City of Grand Island, Hall County, Nebraska.
<br />
<br />3. Materials were furnished and used in the course of installation of a Lennox
<br />furnace in said home on the above-described property at the request of Karleen
<br />Sundermeier, owner of said property, and the balance remaining due for said labor and
<br />materials is the sum of Two Thousand Two Hundred Dollars ($2,200.00).
<br />
<br />4. Jerry's Sheet Metal, Heating & Cooling, Inc. claims a lien on the above-
<br />described real estate in the amount of Two Thousand Two Hundred Dollars ($2,200.00),
<br />together with interest thereon at the legal rate.
