<br />THE STATE OF NEBRASKA, To Michael Ruth, Greetings:
<br />
<br />WHEREAS, you were on December 2,1997, duly appointed Conservator of
<br />
<br />Larry D. Ruth, a protected person, pursuant to lawful proceedings of record in this
<br />
<br />Court.
<br />
<br />NOW, THEREFORE, trusting in your fidelity, the Court does by these presents
<br />
<br />constitute and appoint you to be the Conservator of Larry D. Ruth, a protected person,
<br />
<br />and to perform the duties and responsibilities of Conservator as required by law,
<br />
<br />including the following specific areas of authority:
<br />
<br />(1) Protecting his personal effects;
<br />(2) Giving necessary consents, approvals, or releases;
<br />(3) Applying for private or governmental benefits to which he may be entitled;
<br />(4) Instituting proceedings to compel any person liable for the support of the
<br />proposed ward to support him;
<br />(5) Entering into contractual agreements;
<br />(6) Receiving money and tangible property deliverable to him and applying
<br />such money and property to her expenses for room and board, medical
<br />care, personal effects, training, education, and habilitative service; and
<br />(7) Investing, reinvesting, distributing, selling, conveying, leasing, and
<br />managing his assets, whether they are real, personal or mixed.

<br />These Letters shall constitute certificate and proof of authority of the above-
<br />
<br />named Conservator in all matters involving the incapacitated\protected person.
<br />
<br />You are required to file an Inventory within 90 days after your appointment and
<br />
<br />annual accountings must be filed every 12 months after your appointment. You are
<br />
<br />also required to report the condition of your ward every year and as may be required by
<br />
