Golus, being first duly sworn <br />upon oath, deposes and states as follows: <br />1. That she is the surviving spouse of Clarence T. <br />Galus, who died on the 14th day of April, 1988, a resident <br />of Hall County, Nebraska. That at the date of death, the <br />undersigned Affiant and the above named decedent were husband <br />and wife. <br />2. That a copy of the death certificate for Clarence <br />T. Golus certified by the Director, Bureau of Vital Statistics, <br />Nebraska State Department of Health, to be true and correct, <br />is attached hereto. <br />3. That all interests in property, both real and personal, <br />owned by Clarence T. Galus on the date of death, pass to <br />the undersigned surviving spouse by Will or in the manner set <br />forth in Section 77-2002 R.R.S. Neb.
