<br />WHEREAS, the undersigned, Five Points Bank, a Nebraska Corporation, as Trustee, under the
<br />Trust Deed dated October 22,2001 and executed by Mid Plains Center for Behavioral Healthcare Serv
<br />ices, me. a Nebraska Corporation, as Trustor, in which Five Points Bank, a Nebraska Corporation is
<br />named as Beneficiary, and the undersigned as Trustee, which was recorded as mstrument #200110815 in
<br />the Register of Deeds office of Hall County, Nebraska on the 25th day of October, 2001,
<br />
<br />WHEREAS, said undersigned, Five Points Bank, as Trustee, has received from Beneficiary, a
<br />written request to reconvey the real estate described in the Deed of Trust above mentioned as follows:
<br />
<br />Lots Three (3), Four (4) and Five (5) in Block Eleven (11) in the Original town, Now City of
<br />Grand Island, Hall County, Nebraska
<br />
<br />said request to reconvey reciting that all sums secured by such Deed of Trust have been fully paid;
<br />
<br />NOW THEREFORE, in accordance with such request and the provisions of such Deed of Trust,
<br />the undersigned as Trustee, does hereby reconvey, without warranty, to the person or persons legally
<br />entitled thereto, the estate now held by said Trustee hereunder.
<br />
<br />IN WITNESS WHEREOF, the undersigned has executed this Deed of Reconveyance, at Grand
<br />Island, Nebraska this 16th day of February, 2007.
