<br />For value received, the undersigned hereby grants, assigns and transfers to First
<br />State Bank, 914 Lake Street, P.O. Box 79, Gothenburg, Nebraska 69139, all beneficial
<br />interest under that certain Deed of Trust dated December 20,2006, executed by Scott
<br />Dugan, President, Mid Plains Center for Behavioral Healthcare Services, Inc., Trustor, to
<br />Five Points Bank, Trustee, and recorded as Instrument No. 0200611429 in the office of
<br />the Hall County Register of Deeds describing land therein as follows:

<br />A Tract of land comprising a part of Lot Two (2), Golden Age Second
<br />Subdivision, Grand Island, Hall County, Nebraska, more particularly
<br />described as follows: Beginning at the Northwest comer of said Lot Two
<br />(2); thence easterly along the north line of said Lot Two (2), a distance of
<br />Three Hundred Seventy-two and Sixty-nine Hundredths (372.69) feet;
<br />thence deflecting right 135037'20" and running southwesterly, a distance
<br />of Five Hundred Eleven and Fifty-five Hundredths (511.55) feet, to the
<br />west line of said Lot Two (2); thence northerly along the west line of said
<br />Lot Two (2), a distance of Three Hundred Fifty-seven and Eight Tenths
<br />(357.8) feet to the place of beginning;

<br />together with Promissory Note No. 101211341, described therein or referred to the
<br />money due and to become due thereon with interest, and all rights accrued or to accrue
<br />under such Deed of Trust. The assignment of said note is without recourse.
