200700871 Durable Power of Attorney for Paul R. L. Sensenbach Creation of Durable Power of Attorney By signing this document, I, Paul R. L. Sensenbach, intend to create a durable power of attorney. This durable power of attorney shall not be affected by my subsequent disability or incapacity, and shall remain effective until my death, or until revoked by me in writing. Effective Date This durable power of attorney shall become effective only in the event that I become incapacitated or disabled so that I am not able to manage my financial affairs in which case it shall become effective as of the date of the written statement by a physician, as provided in under "Determination of Incapacity" below. If the durable power of attorney becomes effective, it shall remain effective during any period when I am incapacitated or disabled until my death, or until revoked by me. Detennination of Incapacity The determination of whether I have become incapacitated or disabled so that I am not able to manage my financial affairs shall be made in writing by a licensed physician; if possible, the physician shall be............~.....m...................m.................m....................... ............. ..........._.._................................. Sensenbach, as my attorney in fact. If Katherine P. Sensenbach is unable or unwilling to so serve, I appoint Carolyn Urbom of 652 S. Cherry, Grand Island, NE 68801, and whose telephone number is (308)384-9575, as my attorney in fact, to act for me and in my name and for my use and benefit. Should Carolyn Urbom for any reason fail to serve or cease to serve as my attorney in fact, I appoint Paul R. L.
