<br />The undersigned, being first duly sworn upon oath, deposes and states as follows:

<br />1. That she is an employee of Advantage Title Services, LLC, doing business as a
<br />title and escrow closing company in Grand Island, Nebraska.
<br />
<br />2. That as a part of her duties, she prepare deeds of conveyance in connection with
<br />the closing of transactions involving said title company.
<br />
<br />3. That she did, on or about January 23,2006, prepare a Warranty Deed in which one
<br />Henry G. Leu, Jr. a/kla Henry J. Leu Jr., and Joyce E. Leu, as Grantors, which purported to convey to
<br />HazelL. Struble, as Grantee, the following described real estate located and situated in Hall County,
<br />Nebraska, to-wit:
<br />
<br />The Westerly Fifty-five (55) feet of Lot Four (4), in Block One Hundred Thirty-eight
<br />(138), in Union Pacific Railway Company's Second Addition to the City of Grand Island,
<br />Hall County, Nebraska.
<br />
<br />4. That the Deed as prepared reflected therein the Westerlv Fiftv-six (56) feet of said
<br />
<br />Lot Four (4).
<br />
<br />5. The undersigned recorded the aforesaid Deed on February 16, 2006, as Instrument
<br />No. 200601371 and the deed as recorded reflects the Westerly Fifty-six (56) feet of said Lot Four (4) when
<br />in fact it should read the Westerly Fifty-five (55) feet of Lot Four (4).
<br />
<br />6. The correct legal description is as set forth in paragraph 3 herein and that any
<br />references to Fifty-six (56) feet were made in error by the undersigned as said scrivener.
<br />
