SENSENEY, is one (1) of five (5) living adult children of JUNE F. SENSENEY. 2. That ARTHUR N. SENSENEY is the spouse of JUNE F. SENSENEY. 3. That the undersigned, being one of the seven (7) persons consisting of the spouse, adult children and personal physician of JUNE F. SENSENEY, does hereby determine that JUNE F. SENSENEY is unable to give prompt and intelligent consideration to business matters and as such is deemed to be unable to act from this date forward as the trustee ofthe JUNE F. SENSENEY REVOCABLE TRUST DATED JANUARY 26, 1999, and as amended on February 13, 1999, pursuant to the terms and conditions of paragraph (e) of ARTICLE FIFTH of said trust. 4. That before determining such inability to exist, the undersigned, or one of the living adult children of JUNE F. SENSENEY, first gave notice in writing to JUNE F. SENSENEY, a copy of which is attached hereto and made a part hereof by this reference. 5. That the undersigned, being one of the seven (7) persons consisting of the spouse, adult children and personal physician of ARTHUR N. SENSENEY, does hereby determine that ARTHUR N. SENSENEY is unable to give prompt and intelligent consideration to business matters and as such is deemed to be unable to act from this date forward as the trustee of the JUNE F. SENSENEY REVOCABLE TRUST DATED JANUARY 26,1999, pursuant to the terms and conditions of paragraph (e) of ARTICLE FIFTH of said trust. 6. That before determining such inability to exist, the undersigned, or one of the living adult ,children of ARTHUR N. SENSENEY, first gave notice in writing to ARTHUR N. SENSENEY, a copy of which is attached hereto and made a part hereof by this reference. 7. That all persons or entities paying, delivering, transferring, or issuing property or the evidence thereof on the basis of this Affidavit are hereby indemnified and held harmless by the undersigned, DAVID A.
