That the undersigned, KAREN E. ISA, is one (1) of five (5) living adult <br />children of ARTHUR N. SENSENEY. <br />2. That JUNE F. SENSENEY is the spouse of ARTHUR N. SENSENEY. <br />3. That the undersigned, being one of the seven (7) persons consisting of the <br />spouse, adult children and personal physician of ARTHUR N. SENSENEY, does hereby <br />determine that ARTHUR N. SENSENEY is unable to give prompt and intelligent <br />consideration to business matters and as such is deemed to be unable to act from this date <br />forward as the trustee of the ARTHUR N. SENSENEY REVOCABLE TRUST DATED <br />JANUARY 26, 1999, and as amended on February 13, 1999, pursuant to the terms and <br />conditions of paragraph (e) of ARTICLE FIFTH of said trust. <br />4. That before determining such inability to exist, the undersigned, or one of the <br />living adult children of ARTHUR N. SENSENEY, first gave notice in writing to ARTHUR N. <br />SENSENEY, a copy of which is attached hereto and made a part hereof by this reference. <br />5. That the undersigned, being one of the seven (7) persons consisting of the <br />spouse, adult children and personal physician of JUNE F. SENSENEY, does hereby <br />determine that JUNE F. SENSENEY is unable to give prompt and intelligent consideration <br />to business matters and as such is deemed to be unable to act from this date forward as <br />the trustee of the ARTHUR N. SENSENEY REVOCABLE TRUST DATED JANUARY 26, <br />1999, pursuant to the terms and conditions of paragraph (e) of ARTICLE FIFTH of said <br />trust. <br /> <br />6. That before determining such inability to exist, the undersigned, or one of the <br /> <br />living adult children of JUNE F. SENSENEY, first gave notice in writing to JUNE F. <br /> <br />SENSENEY, a copy of which is attached hereto and made a part hereof by this reference. <br /> <br />7. That all persons or entities paying, delivering, transferring, or issuing property <br />or the evidence thereof on the basis of this Affidavit are hereby indemnified and held <br />harmless by the undersigned, KAREN E. ISA, and are fully authorized to accept this <br />Affidavit and act upon it without any further proof or evidence. <br /> <br />1 <br />
