C(,I/' ) <br />I <br /> <br />The undersigned, being first duly sworn, deposes and says: <br />1. That the undersigned, GORDON J HRNICEK, is personal physician of ARTHUR <br />N. SENSENEY. <br />2. That JUNE F. SENSENEY is the spouse of ARTHUR N. SENSENEY. <br />3. That the undersigned, being one of the seven (7) persons consisting of the spouse, <br />adult children and personal physician of ARTHUR N. SENSENEY, does hereby determine that <br />ARTHUR N. SENSENEY is unable to give prompt and intelligent consideration to business <br />matters and as such is deemed to be unable to act from this date forward as the trustee of the <br />ARTHUR N. SENSENEY REVOCABLE TRUST DATED JANUARY 26, 1999, and as amended <br />on February 13, 1999, pursuant to the terms and conditions of paragraph (e) of ARTICLE FIFTH of <br />said trust. <br />4. That before determining such inability to exist, the undersigned, or one of the living <br />adult children of ARTHUR N. SENSENEY, first gave notice in writing to ARTHUR N. <br />SENSENEY, a copy of which is attached hereto and made a part hereof by this reference. <br />5. That the undersigned, being one of the seven (7) persons consisting of the spouse, <br />adult children and personal physician of JUNE F. SENSENEY, does hereby determine that .JUNE <br />F. SENSENEY is unable to give prompt and intelligent consideration to business matters and as <br />such is deemed to be unable to act from this date forward as the trustee of the ARTHUR N. <br /> <br />SENSENEY REVOCABLE TRUST DATED JANUARY 26, 1999, pursuant to the terms and <br />conditions of paragraph (e) of ARTICLE FIFTH of said trust. <br />6. That before determining such inability to exist, the undersigned, or one of the living <br />adult children of JUNE F. SENSENEY, first gave notice in writing to JUNE F. SENSENEY, a <br />copy of which is attached hereto and made a part hereof by this reference. <br />7. That all persons or entities paying, delivering, transferring, or issuing property or the <br />evidence thereof on the basis of this Affidavit are hereby indemnified and held harmless by the <br />undersigned, KAREN E.
