KALLOS <br /> <br />WHEREAS, you are hereby duly appointed Guardian and Conservator for Nickie John <br />Kallos, Sr., an incapacitated person, to perform the duties and responsibilities of Guardian and <br />Conservator as required by law. <br />The Court finds by clear and convincing evidence that the powers listed below should be <br />granted to the Guardian and Conservator to compensate for the lack of capacity of the ward to <br />manage for himself. Specific Guardianship and Conservatorship powers being authorized are as <br />follows: <br /> <br />A. The power to select the ward's place of abode; <br />B. The power to arrange for the ward's medical care; <br />C. The power to protect the personal effects of the ward; <br />D. The power to give necessary consents, approvals, or releases on behalf ofthe <br />ward; <br />E. The power to arrange for rehabilitative services for the ward; <br />F. The power to apply for private or governmental benefits to which the ward may be <br />entitled; <br />G. The power to institute proceedings to compel any person under a duty to support <br />the ward or to pay sums for the welfare of the ward to perform such duty; <br />H. The power to enter into contractual arrangements on behalf ofthe ward; and <br />I. The power to receive money and tangible property due to the ward and to make <br />expenditures thereof for room and board, medical care, personal effects, training, <br />education and rehabilitative services for the ward. <br /> <br />-1- <br />
