200315114 <br />Each parent shall be entitled to complete and full information <br />from any teacher or school giving instructions to the child or <br />which the child may attend and to have copies of reports given by <br />them, or any of them to a parent. The parent requesting such <br />information shall be responsible for obtaining the same. <br />Upon the death of either one of the parties, the other shall <br />have the custody of the child. <br />Each parent shall have the primary responsibility for meeting <br />the basic needs of the child during the time that the child is with <br />such parent. Both parents recognize the need to develop and <br />maintain consistent standards with respect to diet, behavior and <br />discipline, moral guidance, secular and religious education, <br />personal hygiene and grooming, dress and the other usual conditions <br />affecting the children for which parents are responsible. When the <br />child is in the personal physical custody of a parent, that parent <br />shall have the responsibility for properly caring for the child, <br />including meeting medical and dental emergencies. Both parents <br />shall have the right to make major decisions affecting the child, <br />including but not limited to authorization for major medical, <br />dental, institutional, psychiatric, or other care, schooling and <br />college selection, and inspection and receipt of medical records. <br />The permission of both parents shall not be necessary in the event <br />of a medical or dental emergency but they shall further consult and <br />agree on all non - emergency health care and on all school and <br />college selections. <br />The parties will cooperate with each other in carrying out the <br />3 <br />
