200314087 <br />obligation shall be $1,029 per month. This support will continue until the minor child Abby <br />reaches 19 years, becomes emancipated, becomes self - supporting, marries, or dies, or until <br />further order of this Court. Petitioner shall claim the minor child Jordan as an exemption for <br />tax purposes and Respondent shall claim the minor child Abby for tax purposes. Child <br />support is calculated based on the average income of Petitioner for the years 1996 through <br />1999. No tax returns for the Petitioner or the corporation for the year 2000 were available <br />at the time of trial. While the financial statement for the corporation appears to show <br />decreased earnings, the court is without information to adequately figure Petitioner's income <br />past 1999. <br />4. The Petitioner shall maintain health insurance on the minor children, and each <br />party shall pay one -half of any unreimbursed medical, dental, optical, and orthodontia <br />expenses for each of the children. <br />5. The Petitioner's obligation to pay child support, medical support, and alimony <br />shall be subject to income withholding. The Petitioner shall immediately prepare a Notice <br />to Withhold Income and deliver said Notice to the Petitioner's employer and provide a copy <br />to the Clerk of the District Court and to the Respondent. <br />6. Each party shall continue to have full and equal access to the education and <br />medical records of the minor child. <br />7. Either party may make emergency decisions affecting the health or safety of the <br />minor child while the child is in the physical custody of such parent. <br />
