200505378 <br />(b) Each party shall promptly inform and consult with the other in the <br />event of any serious medical problems with the minor child of the <br />parties. <br />(c) Each of the parties shall have access to information relating to the <br />minor child including, but not limited to, access to school, <br />governmental, law enforcement and medical records, and access to <br />all teachers, governmental officials, law enforcement officers, and <br />doctors, and other professionals having contact with the minor child. <br />Such information and contact to be available to each parent without <br />notice to or any further consent of the other. <br />(d) Each parent is authorized to consent to emergency medical care of <br />the minor child at the times when the other parent is not reasonably <br />accessible to give that consent. <br />The parties shall inform each other of the child's social and educational activities and <br />appointments so that both parents might participate where possible and appropriate, and <br />each party shall further advise the other of the children's emergency situations, illnesses, and <br />like problems that may occur when the child is in his or her respective care. <br />The Petitioner shall pay through the Nebraska Child Support Enforcement Office <br />child support for the support and maintenance of the minor child in the amount of $85.00 <br />per month; said payments shall commence upon the first day of January, 2004, and continue <br />to be due and payable on the first day of each month thereafter, until the minor child reaches <br />the age of nineteen, becomes emancipated, becomes self-supporting, marries, dies, or further <br />order of the Court. <br />IT IS FURTHER ORDERED that the Petitioner be and hereby is required to furnish <br />the HALL COUNTY DISTRICT COURT with his address, telephone number, social <br />security number, name of his employer and any other information the Court shall deem <br />relevant until such judgment shall be paid in full. Petitioner shall be required to advise <br />the HALL COUNTY DISTRICT COURT of any changes in such information between <br />the time of entry of the Decree and the payment of the judgment in full. Failure to <br />comply with this provision shall be punishable by contempt. <br />In the event Petitioner fails to pay any such child support payment in an. amount <br />equal to the support due and payable for a one -month period of time, he shall be subject to <br />income withholding and may be required to appear before this Court on a date to be <br />
