<br />Mike Steinke, being first duly sworn upon oath, deposes and says:

<br />1. He verifies this account in writing for and on his behalf doing business as Steinke
<br />Electric.
<br />
<br />2. Shafer Development and Cedar Ridge Development, LLC by and through Joel Shafer,
<br />entered into a verbal contract with Mike Steinke, d/b/a Steinke Electric to provide labor and
<br />material for electrical work to be placed upon the real estate hereinafter described.
<br />
<br />3. The material and labor was provided on real estate located at 1203 and 1205 Cedar
<br />Ridge Ct., Grand Island, NE 68803, which real estate is legally described as follows, to-wit:
<br />
<br />Lot One (1), Cedar Ridge Second Subdivision to the City of Grand Island, Hall
<br />County, Nebraska.
<br />
<br />4. The date that the last materials or services were last furnished to said real estate was
<br />on or about August 4,2005, and the remaining unpaid contract price for these services is Three
<br />Thousand Two Hundred Fifty and No/IOO Dollars ($3,250.00).
<br />
<br />5. Mike Steinke, d/b/a Steinke Electric, claims a lien on the above-described real estate
<br />for the full amount of the account due of Three Thousand Two Hundred Fifty and No/l 00
<br />Dollars ($3,250.00), together with interest thereon at the legal rate as allowed and provided by
<br />law, from and after August 4, 2005.
