<br />and as said President verifies this account in writing for and on behalf of said corporation.
<br />2. Middleton Electric, Inc., by and through its duly authorized officers, entered into a
<br />contract with Lacy Construction, provide labor and materials for the electrical requirements
<br />relating to construction of a home upon the real estate hereinafter described and the contract
<br />price for the improvements to be constructed upon such real estate by Middleton Electric, Inc.
<br />including change orders through March 24, 2003 is Sixteen Thousand Six Hundred Eight and
<br />24/100 Dollars ($16,608.24).
<br />3. The labor provided to date and to be provided in the future is upon and in respect to
<br />that real estate located at Grand Islanfd, Nebraska, legally described as follows, to -wit:
<br />Lot Three (3), Block Two (2), Crane Valley Subdivision, in the City of
<br />Grand Island, Hall County, Nebraska.

<br />4. The amount unpaid and due to Middleton Electric, Inc. for materials and services
<br />provided in accordance with the invoices and change orders through March 24, 2003 at Sixteen
<br />Thousand Six Hundred Eight and 24/100 Dollars ($16,608.24) together with interest thereon as
<br />provided by applicable law.
