<br />TOM MIDDLETON, duly elected President of Middleton Electric, Inc., a Nebraska
<br />corporation, being first duly sworn upon oath, deposes and says:
<br />1. He is the duly elected President of Middleton Electric, Inc., a Nebraska corporation
<br />and as said President verifies this account in writing for and on behalf of said corporation.
<br />2. Middleton Electric, Inc., by and through its duly authorized officers, entered into a
<br />contract with Snyder Construction Co., provide labor and materials for the electrical
<br />requirements relating to construction of commercial building upon the real estate hereinafter
<br />described and the contract price for the improvements to be constructed upon such real estate by
<br />Middleton Electric, Inc. including change orders through March 25, 2004 is Twelve Thousand
<br />One Hundred Seventy -Three and No /100 Dollars ($12,173.00).
<br />3. The labor and materials provided is upon and in respect to that real estate located at
<br />Crane View Center, 810 North Diers Avenue, Suite J, Grand Island, Nebraska, being a part of
<br />that property legally described as follows, to -wit:
<br />Lot Three (3), Block Two (2), Crane Valley Subdivision, in the City of
<br />Grand Island, Hall County, Nebraska.
<br />4. The amount unpaid and due to Middleton Electric, Inc. for materials and services
<br />provided in accordance with the invoices and change orders through March 25, 2004 at Twelve
<br />Thousand One Hundred Seventy -Three and No /100 Dollars ($12,173.00) together with interest
<br />thereon as provided by applicable law.
