<br />materials and skill. and improvements is true and correct: that same were done and performed and
<br />furnished by the undersigned for the said,'k _ under a contract for
<br />the installation of septic tank, leech field, and plumbing fixtures on the following lot, piece or parcel
<br />of land. vis _6600 Wildwood Drive, Alda, Nebraska Lot Four (4) A -G Acres Third Subdivision, an
<br />Addition to the village of Alda, Hall County Nebraska
<br />That at the time said contract was made and labor and materials furnished and delivered thereunder
<br />Joseph and Kimberly Ignowski were the owner of said premises.
<br />That the date of the first item furnished and delivered was _July 17, 2003
<br />and the date of the last item was November 24, 2003
<br />Affiant further states that said labor was performed on, and materials were furnished for, delivered at.
<br />and used in said building or premises on and between the dates specified.
<br />That the prices charged therefore are fair and reasonable, and that there is now due on said account the
<br />sum of Five thousand four hundred thirty-one dollars and forty -four cents that said claims a lien on
<br />the said premises for the full amount of said account, to wit: The sum of _$5112.56 together
<br />with interest thereon at the legal rate from the 18 day of _February_, 2004_ and
<br />further affiant says not.
