The Grand Island Streets Division shall be contacted prior to work beginning at 308-385-5322. A <br />Notice to Proceed with work in the ROW will not be given until a pre -construction notice has been <br />given; the City of Grand Island permit number is required when requesting the Notice to Proceed. <br />Any keyhole potholing locations in the roadway shall be approved by the Streets Division and shall <br />be filled within five days of potholing. No handholds or vaults shall be placed in the flowline or <br />sloped bank of drainage ditches. If it has to be placed on sloped bank the final grade of hand hold <br />or vault lid shall match that of the sloped bank. Erosion control shall be in place for all open <br />excavations adjacent to drainage ditches. All drainage ditch flowlines must be restored before job <br />is done. No handholds, vaults, or manholes shall be placed in the ADA accessible area of the <br />sidewalk. Any handholds, vaults or manholes placed in the sidewalk shall match the existing <br />slopes of the sidewalk and shall not create a tripping hazard for pedestrians. Any sidewalk <br />removals & replacements shall be approved by the City of Grand Island prior to removal. <br />Restoration of sidewalk shall be done within 5 days of removal and shall be inspected by the City of <br />Grand Island Public Works Department. Any street or sidewalk closures require a 72-hour notice to <br />the City of Grand Island Public Works Department at 308-385-5444. Contractor will be required to <br />request a locate before any physical work is to take place. Locate flags shall be removed after the <br />"excavator" has determined the work is complete. All ROW shall be restored to the same condition <br />as prior to the work taking place which includes but is not limited to placement of top soil & <br />seeding at excavated areas within green space and repairing washouts or ruts from construction, <br />seeding disturbed earth, and seeding & erosion control on sloped banks after disturbed. A ROW <br />inspection shall be completed once work is done. Contact the Grand Island Streets Division at 308- <br />385-5322 to request an inspection. Where CROSSING SIGNAL INFRASTRUCTURE Conduit shall not <br />be placed between the City's traffic signal cabinet and connecting traffic signal conduit/poles. <br />Conduit must be offset a minimum of 5' from the base of any traffic signal cabinet, traffic signal <br />pole, or traffic signal pull box. IF SIGNAL CONDUIT/WIRE HAS TO BE CROSSED THE NEW LINE <br />SHALL BE A MINIMUM OF 6' DEEP WHERE CROSSING AND THE EXSTING SIGNAL CONDUIT SHALL <br />BE POTHOLED AND VERIFIED ON -SITE PRIOR TO BORING. <br />
